Application for Employment Services

You must complete these two forms to apply for employment services. The information provided will enable Ministère personnel to identify the specific actions needed in order for you to enter and remain in the work force. Submit the duly completed forms in person or by mail to your nearest local employment centre.

Application for Last-Resort Financial Assistance  

You must complete these three forms in order to apply for last-resort financial assistance. If the application is for a couple or family, each adult must complete an Application for Service – General Information (3003A) (dynamic, 386 KB) and Appendix 1 – Application for Employment Services (3003-01A) (dynamic, 404 KB) . However, only one Appendix 2 – Application for Last-Resort Financial Assistance (3003-02A) (dynamic, 195 KB ) needs to be completed per couple or family. Submit the duly completed forms in person or by mail to your nearest local employment centre (CLE).

Important: Your eligibility for last-resort financial assistance is determined as of the date on which your local employment centre receives the forms.
